Understanding Challenges in Adult Autism Diagnosis and Support

Receiving an autism diagnosis in adulthood and accessing subsequent support services often involves a complex and prolonged journey. A recent mixed-methods systematic review has synthesized perspectives from adults diagnosed with autism, their family members, and clinicians to better understand the obstacles encountered during diagnosis and post-diagnostic care.

Study Overview and Scope

The review incorporated data from 50 studies, encompassing 4,487 adults diagnosed with autism, 902 self-identifying autistic individuals, 217 family members, and 198 clinicians. The research team conducted comprehensive searches across databases including Web of Science, PsycINFO, Embase, Medline, and ProQuest Dissertations & Theses. The quality of included studies was assessed using established tools such as the Joanna Briggs Institute instruments and the Delphi Critical Appraisal Tool.

Key Themes Identified

Through thematic synthesis, three major themes emerged:

  • Recognition and Decision-Making: Challenges in identifying autism traits and deciding to seek diagnosis.
  • Negotiation at the Interface: Interactions and communication between adults seeking diagnosis, families, and clinicians.
  • Resistance and Resolutions: Conflicts and eventual strategies to overcome barriers.

The study highlighted tensions among stakeholders, particularly regarding access to information, the use of diagnostic tools, the role of self-identification, and complications arising from co-occurring mental health conditions.

Impact on Gender-Diverse and Ethnic Minority Individuals

The review noted that barriers to diagnosis and support were often intensified for gender-diverse and ethnic minority adults. However, reporting on ethnicity and gender identity was limited across the studies, indicating a need for more inclusive research practices.

Implications for Research and Practice

Differences in how various stakeholders interpret diagnostic practices and related barriers may themselves obstruct timely access to services. The authors recommend transparent demographic reporting and enhanced availability of information about diagnostic and support services to improve outcomes for adults seeking autism diagnosis.

Cautions and Limitations

While the findings provide moderate to high confidence in identified themes, the scarcity of data on marginalized groups limits the generalizability of some conclusions. Further research is needed to explore these perspectives more fully.
