Overview of Recent Research on Paracetamol Use During Pregnancy

A comprehensive umbrella review has examined whether taking paracetamol (acetaminophen) during pregnancy is associated with a higher risk of autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) in children. This analysis combined data from multiple systematic reviews and meta-analyses to assess the strength and reliability of existing evidence.

Key Findings from the Meta-Meta-Analysis

The review incorporated seven meta-analyses, including data from over three million participants. Initial analyses without restrictions indicated a small but statistically significant association: prenatal paracetamol exposure was linked to a 17% increased risk of autism and a 29% increased risk of ADHD in offspring.

However, when the researchers limited the analysis to higher-quality studies and those with a low risk of bias, these associations weakened considerably. In several models, the estimated risks approached no increased risk at all. The study also noted considerable variability among the included studies and overlapping data sets, which complicates interpretation.

Context and Interpretation

The authors highlighted that methodological issues such as residual confounding factors and inaccuracies in measuring paracetamol exposure likely influenced the observed associations. These limitations mean that the current epidemiological evidence does not support a definitive causal link between prenatal paracetamol use and neurodevelopmental disorders.

Implications for Pregnant People and Healthcare Providers

Given the findings, paracetamol remains a recommended first-line medication for managing pain and fever during pregnancy when clinically necessary. Pregnant individuals should continue to consult healthcare professionals about medication use, balancing benefits and potential risks.

Limitations and Cautions

The review underscores the need for cautious interpretation of observational data in this area. High heterogeneity among studies and overlapping participant samples reduce the certainty of conclusions. Further high-quality research is needed to clarify any potential risks.
