Study Finds Changing Profiles of Children Diagnosed With ADHD and ASD Over Time

Introduction

Recent research indicates that the characteristics associated with diagnoses of attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorder (ASD) in children and adolescents have evolved over the past decade. A large Danish population-based study examined whether the prediagnostic factors linked to these neurodevelopmental conditions have shifted from 2012 to 2022.

Key Findings

The study analyzed data from over 2.1 million individuals born between 1994 and 2022, focusing on 71,317 children and adolescents diagnosed with ADHD or ASD before age 18 during 2012–2022. Each diagnosed individual was matched with ten peers without these diagnoses based on sex, birth year, municipality, and country of birth.

Researchers assessed 19 prediagnostic characteristics, including family socioeconomic status, birth and perinatal factors, and healthcare usage. While those diagnosed with ADHD or ASD initially showed notable differences from their peers across these factors, these differences decreased over time. For example, the odds ratio (OR) associating low birth weight with diagnosis declined from 1.54 in 2012–2013 to 1.17 in 2020–2022.

The attenuation of associations was more pronounced for ADHD than ASD and was especially evident in individuals diagnosed between ages 10 and 17. The trends were consistent across sexes and remained stable when analyses were limited to cases with higher diagnostic certainty.

Context and Interpretation

These findings suggest that the rising rates of ADHD and ASD diagnoses may be influenced more by changes in diagnostic criteria, healthcare accessibility, and awareness rather than by shifts in the underlying risk factors alone. The increasing similarity between diagnosed individuals and their peers on socioeconomic and perinatal characteristics points toward broader diagnostic inclusion over time.

Implications for Families and Clinicians

Families and healthcare providers should understand that the growing number of ADHD and ASD diagnoses does not necessarily imply that these conditions are becoming less severe or impairing. Instead, evolving diagnostic practices may capture a wider spectrum of individuals. This underscores the importance of personalized assessment and support tailored to each child’s needs.

Limitations and Cautions

While the study provides valuable insights, it is observational and cannot establish causation. The data are specific to Denmark and may not fully generalize to other populations. Additionally, changes in diagnostic tools and healthcare policies over time could influence findings.
