Early Childhood General Anesthesia Linked to Slightly Increased Risk of Neurodevelopmental Disorders

Study Examines Long-Term Neurodevelopmental Risks of Early Anesthesia

A recent nationwide cohort study from South Korea has explored the relationship between exposure to general anesthesia (GA) before the age of three and the subsequent diagnosis of neurodevelopmental disorders (NDDs) through adolescence. The research analyzed data from over one million children born between 2005 and 2007, comparing those who underwent GA early in life with those who did not.

Key Findings

After matching 131,699 children based on propensity scores to control for confounding factors, the study found that early exposure to GA was associated with a modestly increased risk of developing NDDs later in childhood. Specifically, the hazard ratio (HR) for any NDD diagnosis was 1.19 (95% confidence interval [CI], 1.12–1.26), indicating a 19% higher risk compared to unexposed peers.

The strongest associations were observed for intellectual disability (HR 1.56; 95% CI, 1.32–1.85) and autism spectrum disorder (HR 1.46; 95% CI, 1.17–1.82).

Context and Methodology

Previous animal studies have suggested that GA may have neurotoxic effects on the developing brain, but clinical evidence has been inconsistent. Many prior studies had limited follow-up periods or focused narrowly on specific neurodevelopmental outcomes. This study addressed these gaps by using a large, population-based dataset with extended follow-up into adolescence and by adjusting for surgical complexity using Total Relative Value Units (TRVUs) as a proxy.

Implications for Families

While the findings suggest a slight increase in risk, the overall absolute risk of neurodevelopmental disorders remains low. Families facing the need for surgery requiring GA in early childhood should discuss the benefits and risks with their healthcare providers. Decisions about anesthesia should consider the medical necessity of the procedure alongside these findings.

Limitations and Considerations

The study authors caution that unmeasured confounding factors related to surgery and underlying health conditions cannot be fully excluded. The associations observed do not prove causation but highlight the need for further research to clarify mechanisms and identify potential protective strategies.
