New Framework Links Autism, ADHD, Dyslexia, and Other Neurodivergent Profiles Through Shared Neural Architecture

Reframing Neurodivergence with a Unified Neural Model

A recent conceptual review proposes a novel framework that connects various neurodevelopmental conditions—such as autism, attention-deficit/hyperactivity disorder (ADHD), developmental coordination disorder (DCD), dyslexia, and profiles of high ability—under a common neurobiological architecture. This approach, called the High Processing Cost Phenotypes (HPCPh) framework, suggests these diverse presentations arise from shared underlying brain development patterns rather than being entirely separate clinical entities.

Key Features of the HPCPh Framework

The HPCPh model builds on prior research into atypical neural topology, emphasizing the role of the brain’s extracellular matrix (ECM) in shaping neural connectivity. It proposes that a more flexible but less stabilized neural structure leads to a characteristic spatial organization within the brain, described as the Island-Desert-Bridge (IDB) topology.

  • Islands: Regions of high efficiency that excel at local information processing and integration.
  • Deserts: Areas with weaker stabilization involved in sequential processing, timing, and sensorimotor coordination.
  • Bridges: Energetically costly pathways that connect Islands and Deserts, facilitating communication across these domains.

Explaining Clinical Paradoxes in Neurodivergence

This IDB topology helps explain seemingly contradictory traits often observed in neurodivergent individuals. For example, a person might demonstrate exceptional sensory skills yet experience sensory overload, or show strong intentions but struggle with motor execution (dyspraxia). Similarly, semantic knowledge may be rich while reading ability is limited, or intense focus on salient stimuli might coexist with difficulties in routine tasks.

Thermodynamics and Functional Costs

The framework also introduces a thermodynamic perspective on cognitive fatigue and mood-related symptoms. It suggests that when the metabolic and autonomic demands of maintaining the “Bridges” exceed the brain’s tolerance, there can be a reduction in functional capacity, manifesting as burnout or depression-like states. This concept, termed Functional Iron Blockade (FIB), is a hypothesis that requires further empirical validation and does not exclude the presence of primary mood disorders.

Implications for Families and Clinicians

By shifting focus from isolated symptoms to the underlying neural topology and processing costs, the HPCPh framework offers a transdiagnostic lens to understand neurodivergent profiles. This perspective may encourage more integrative approaches to support, emphasizing the shared neurobiological mechanisms rather than discrete diagnostic categories.

Limitations and Future Directions

While promising, this model is currently conceptual and requires empirical testing to validate its hypotheses and clinical utility. It also does not replace existing diagnostic frameworks but rather aims to complement them by linking neurodevelopmental phenotypes to developmental biology and systems neuroscience.
