Screening Tool Shows Promise for Detecting Psychosis Risk in Autistic Adults: Study from the Netherlands Autism Register

Study Evaluates Prodromal Questionnaire-16 in Autistic Adults

Research indicates that autistic adults may experience a higher prevalence of early psychotic symptoms than previously recognized. A recent study assessed the effectiveness of the Prodromal Questionnaire-16 (PQ-16), a brief screening tool, in identifying attenuated psychotic symptoms and predicting schizophrenia-spectrum diagnoses within this population.

Key Findings from a Large National Cohort

The study analyzed data from 1,686 autistic adults enrolled in the Netherlands Autism Register, averaging 46 years of age. Participants completed the PQ-16, which measures early psychotic symptoms and associated distress. Results showed that 34.1% of participants scored above the established cutoff (≥6), suggesting a substantially higher rate of attenuated psychotic symptoms compared to non-autistic groups.

Further analysis revealed that higher PQ-16 scores were independently linked to more severe autistic traits and increased depressive symptoms. Among a subset of 361 individuals with follow-up data, 1.4% reported new-onset schizophrenia-spectrum diagnoses. Notably, all new cases occurred in participants who had initially scored above the PQ-16 cutoff, representing 3.2% of that group compared to none below the cutoff.

Implications for Screening and Clinical Practice

These findings suggest that the PQ-16 may serve as a useful initial screening instrument to detect early psychotic symptoms in autistic adults. The study supports incorporating brief screening into a two-step assessment process to better identify individuals at risk for psychosis within this population.

Considerations and Limitations

While the PQ-16 showed apparent ability to discriminate risk, the small number of new schizophrenia-spectrum cases limits the strength of predictive conclusions. Researchers advise cautious interpretation and recommend further studies to validate these results and refine screening approaches tailored for autistic adults.

Overall, this study highlights the importance of recognizing and monitoring attenuated psychotic symptoms in autistic individuals, which may otherwise be overlooked due to overlapping clinical features.
