Systematic Review Evaluates Valproate and Divalproex in ASD Treatment

A recent systematic review assessed the current evidence regarding the use of valproate (VPA) and divalproex sodium (DVPX) for managing behavioral symptoms in children and adolescents with autism spectrum disorder (ASD). The focus was on symptoms such as irritability, aggression, and repetitive behaviors, which can significantly impact quality of life.

Study Selection and Methods

Researchers conducted a comprehensive search of major medical literature databases to identify randomized controlled trials (RCTs), open-label trials, and other relevant clinical studies involving pediatric patients with ASD treated with VPA or DVPX. From an initial pool of 654 abstracts, four clinical trials met inclusion criteria. Among these, three were RCTs with sample sizes of 13, 27, and 30 participants respectively, and one was an open-label trial.

Findings on Efficacy and Safety

While most included studies reported some improvement in overall symptoms, the trials used varied outcome measures, limiting direct comparisons. Due to differences in study design and measures, a meta-analysis was not feasible. The evidence quality was rated as low, and the review highlighted a lack of sufficient data to support definitive recommendations for using VPA or DVPX to improve core ASD symptoms.

Potential Role in Managing Behavioral Challenges

Despite limited evidence, VPA and DVPX may be considered as off-label options for certain patients exhibiting significant irritability, aggression, or repetitive behaviors associated with ASD. However, clinicians and families should weigh potential benefits against safety concerns and the current lack of robust evidence.

Implications and Future Research

The review underscores the need for larger, well-designed clinical trials to better understand the efficacy and safety profile of valproate and divalproex in the pediatric ASD population. Such studies could inform clearer clinical guidelines and support more personalized treatment approaches.
